For your convenience, all of your patient registration forms will be provided to you electronically, regardless of if you are receiving therapy in person or through online telehealth. This allows us to save paper, while making it easier for you to complete the registration process before your first appointment. Please read below for what you should expect during and after scheduling an appointment with Optimal Recovery & Therapy.
The registration process:
- Schedule an appointment by clicking the red button on this website. From there you will click on the type of appointment you want, location for therapy, time slot that works best for you, and fill in your basic contact and payment information.
- After your appointment request is submitted and approved, you will receive an email within 24 hours from Optimal Recovery & Therapy PLLC with a link to open your patient portal and fill out the required paperwork. Please make sure you complete these forms at least 24 hours before your first appointment.
- After you fill out the paperwork online, including using electronic signatures, your therapist will receive your information and start preparing for your first appointment!
